An AstraZeneca PAP for Seniors Enrolled in Part D

The introduction of the Medicare Part D prescription drug coverage eliminated some PAPs and changed the eligibility of others. These changes are continuing and there is still confusion about PAP eligibility guidelines for Medicare recipients.

AstraZeneca has created the AZ Medicine & Me program specifically for people enrolled in Medicare Part D. Qualified applicants are immediately enrolled in the plan. A permanent card and welcome kit will come arrive in the mail. This card can be presented to the pharmacy when picking up the prescription medicine. The medicine costs no more than $25 for a typical 30-day supply. There is no cost to enroll.

To qualify for the program, you must:

  • Have the Medicare Drug Coverage
  • Be taking any of the AstraZeneca medicines listed below
  • Be an individual with an annual income below $30,000 or a couple earning less than $40,000
  • Have already spent at least 3% of your annual household income this year out-of-pocket on prescription medicines

The program includes these AstraZeneca medicines:

  • Accolate
  • Arimedex
  • Casodex
  • Crestor
  • Nexium
  • Plendil
  • Prilosec
  • Plumicort Turbuhaler
  • Rhinocort Aqua
  • Zomig
  • Zomig-ZMT

To enroll or learn more, call 1-800-957-6285 or visit the website at www.azmedicineandme.com.
